Service

[ 1 ] Parties involved in the current contract: Marie-Pierre Gosselin, M.A. psyc., Mouton d’or Child sleep consultant (hereafter « the Consultant ») residing at 12241 rue Beauséjour, Montreal.
and
"the client".


As of the date of payment by the client, the consultant will provide one of the following packages in regards to the sleep of the child.

Hourly consultation by phone
This package includes:
• A 15-minute initial evaluation by phone
• A basic online evaluation form about the child’s sleep
• A 60-minute phone consultation in regards to the child’s sleep

Hourly face-to-face consultation
This package includes:
• A 15-minute initial evaluation by phone • A basic online evaluation form about the child’s sleep • A 60-minute face-to-face consultation in regards to the child’s sleep at the client’s home.

3-day consultation
This package includes
• A 15-minute initial evaluation by phone
• A detailed online evaluation form about the child’s sleep
• A 60-minute phone consultation or face-to-face consultation (at the client’s home) in regards to the child’s sleep.
• Personalized sleep plan for the child
• A 15-minute phone call scheduled before the sleep plan’s implementation date to obtain clarifications. Date and time for this phone call will be fixed in advance.
• Support service that will start on the first day of the sleep plan implementation and end 3 days after the beginning of the implementation. This support service consists of a shared online sleep log that allows for daily communication with the consultant to follow the child’s progress.

7-day consultation
This package includes:
• A 15-minute initial evaluation by phone • A detailed online evaluation form about the child’s sleep • A 60-minute phone consultation or face-to-face consultation (at the client’s home) in regards to the child’s sleep. Mouton d’or, Marie-Pierre Gosselin M.A. Psych. • Personalized sleep plan for the child • A 15-minute phone call scheduled before the sleep plan’s implementation date to obtain clarifications. Date and time for this phone call will be fixed in advance.  Support service that will start on the first day of the sleep plan implementation and end 7 days after the beginning of the implementation. This support service consists of a) a shared online sleep log that allows for daily communication with the consultant to follow the child’s progress, and b) a 15-minute phone call that will be scheduled in advance (at the end of the 7 days).


[ 2 ] The client will receive the sleep plan via email within 48 hours following the consultation, based on the package selected in paragraph 1. The client will have 24 hours to revise the plan and will have the opportunity to ask questions to the consultant and obtain clarifications via phone (15 minutes maximum) or via email. The client agrees to follow the sleep plan as it is written. The plan will start at a date mutually agreeable to both parties. The consultant reserves the right to terminate the agreement if the Client breaches any of the terms of this agreement.


[ 3 ] The Client will have the option to seek additional support from the Consultant. The cost for additional support is $95, and includes: one 15-minute phone call and 3 days of email support (answered only between 9am-9pm EST). This additional support will expire one week after the payment date.


Health professional

[ 4 ] The Client agrees to consult a family doctor or pediatrician (« health professional ») about the Client’s intention to sleep train and implement the sleep plan with the Client’s child prior to sleep plan implementation. It is the Client’s responsibility to rule out any underlying medical conditions with the Client’s health care practitioner that may be causing sleep problems (i.e. sleep apnea, ear infection, allergies, asthma, etc.), as well as ensure that the health care practitioner has advised the Client’s child is gaining weight appropriately, is thriving, and that it is appropriate to implement the sleep plan and/or cease night feedings. The Client agrees to notify the Consultant of any medical changes during the sleep training process (i.e. illness, ear infection, etc.) as sleep training should not be implemented unless the child has a “healthy” diagnosis from a health care practitioner.


Liability and Disclaimer

[ 5 ] The information provided by the Consultant is not intended, nor is implied to be a substitute for professional medical advice. The Client is advised to always seek the advice of the Client’s health care practitioner or other qualified health care provider with questions regarding medical conditions, or the health and welfare of the Client’s baby, toddler or child. The Consultant will use reasonable efforts to include up-to-date and accurate information in this consult, but makes no representations, warranties, or assurances as to the accuracy, currency, or completeness of the information provided. The Consultant shall not be liable for any damages or injury resulting from the Client’s access to, or inability to access the information discussed, or from the Client’s reliance on any information provided by the Consultant. The consultation may provide references to other materials and resources, but the Consultant will have no responsibility for the content of such other references and shall not be liable for any damages or injury arising from that content. Any references provided by the Consultant are provided merely as a convenience to the Client.


Payment of the Consultant

[ 6 ] A lump sum will be paid to the consultant for her work based on the package described in paragraph 1, and accepted in accordance to the present contract. The Consultant will provide an invoice to the client, and will collect payment in full before the 60-minute consultation with the Client. All sales are final.


Confidential information

[ 7 ] The Consultant agrees that any information received by her as a result of this contract, which concerns the personal, financial, or other affairs of the Client will be treated by the Consultant in full confidence and will not be revealed to any other persons, or organizations, without written consent from the Client. The Client agrees to keep this sleep plan for the Client’s personal use and shall not share the content of the sleep plan with outside parties without written consent, with exception of the Client’s health care practitioner.
